Instructions for filling out proxy vote form
The form which allows owners to vote by proxy in Isla de Oro general assemblies is in Spanish in order to be legally valid. Here is an overview and instructions for filling it out.
The four fields marked with RED are the most important and must be filled in. If you leave the others blank, they will be added at the time of the assembly.
Important fields are:
1. Name of person who signs.
2. Cedula #. If you do not have a Venezuelan cedula, put passport # here instead.
4. Apartment number (one vote form per condo).
11. Signature.
Other fields:
3. Your home town or city.
5. Number of your share in Isla de Oro Sports and Beach Club.
6. Person who votes for you.
7. His or her cedula number.
8. Location where signed.
9. Date of month when signed.
10. Month and year of signing.
12. Cedula number, repeated (may be substituted with passport).
13. If apartment is owned by a corporation, list corporation name here.
14. Title in the corporation of person who signs.

Faxes and photocopies of the form itself are accepted, however all signatures must be originals. (Photocopies or faxes of signatures are not valid).
Note that voting is not secret, due to the fact that voting is entered on a per-apartment basis in a book recording the proceedings of the assemblies.
